Requesting a Personal Sacramental Record
Sacramental records can only be provided to the individual whose name is in the sacramental notation or with the individual’s written permission. Information may be released to parents and guardians if the individual is under 18 or if there is legal documentation that entitles a person to request this information. If the individual is deceased a family member may access their sacramental record with the proper identification.

Sacramental Certificates for Canadian Citizenship (C-3)Due to an overwhelming surge in family history requests for Canadian citizenship purposes we are temporarily suspending the creation of documents. This will only be temporary as we are developing a streamlined system that will be available on our diocesan website that will be activated sometime in April. Please reconnect with us then.
Another resource you may consider to obtain baptism or marriage records is the Archives of Ontario to order a copy of a birth, marriage or death registration - Archives of Ontario. If your Canadian descendant was born after 1869 the Archives of Ontario may also be able to assist with your request.

Individuals seeking a personal sacramental record for government or employer purposes or seeking to obtain their First Nations or Metis status should state the nature of your request.
