Requesting a Personal Sacramental Record
Sacramental records can only be provided to the individual whose name is in the sacramental notation or with the individual’s written permission. Information may be released to parents and guardians if the individual is under 18 or if there is legal documentation that entitles a person to request this information. If the individual is deceased a family member may access their sacramental record with the proper identification.
If you are searching for a personal sacramental record and you are aware of the church where the sacrament was administered but the church has since closed please view the list of suppressed parishes to see where the sacramental record can be located. (Suppressed parish list)
